When doing an interview you should know the names of the people interviewing you. Ma'am is a dismissive term when using it toward someone when you know their name. Pretty minor in the big picture. Being dismissive of women is a cornerstone of Sanders persona.

....and I was taught that ma'am was to be used except, as you point out, if you don't know the woman's name. Otherwise it was considered an insult.

The use of the word "Ma'am" is generally acceptable across the country, just like it's male equivalent, "Sir" is generally acceptable.

If you know their name then it is a slight. If you know their name, even if you don't know them well, you use their name. The only variable at that point being if you use their first name or address them is Ms. or Mr.

In Brooklyn, at least, that was considered a derogatory reference to someone whose name is known.

They know better. And they're not doing themselves any favors by jumping on this bandwagon of dubious origin. Who reported it in the first place? Wasn't this a private fundraiser? I realize there's no such thing as privacy but pretending it was something other than it was is really not helping anyone's credibility.
