Notice in this clip his pressured speech, words tumbling out, and he seems somewhat agitated.

Contrast that with his calmer,slower speech pattern during his lawn chair interview with Steponopolus
last week.

One manifestation of his mental problem is he has to micro-manage, he is famous for it, and can't trust anyone except, maybe, a teeny bit, his kids.
and he's paranoid.
So sleeping even for a few hours is fraught, 'cause he might miss something.

I've had patients who were lke this, to a lesser degree, and they had insight into the problem.

He has zero insight, zero ability to reflect on his behavior.

I cannot even say. I'm guessing that many of his early (mildly sane) cabinet members and WH staff began to recognize how impaired he is and he has gotten rid of all of them except for the ones who choose to use it to their advantage (Steven Miller, Kellyanne Conway, and even Sarah Sanders). The fact is that the R Senators likely know this as well and they continue on as if nothing is wrong. They are all smart enough (OK, maybe not Marco Rubio) to know how dangerous this is, and yet they do nothing. Worse than nothing.

To your last point - insight into the problem - he cannot reflect inward because it scares the shit out of HIM!

Geez.
