They would run as an Independent. This would come with the full knowledge that they were a spoiler and that both they and Trump would go down.
I don't think it will do any good to actually run in a Republican primary. The GOP has lost so many of it's sane voters to Independent status that the only ones left are loyal to Trump. Everyone else has left and typically won't be eligible to vote against him.
