No one else can vote in their place, right? So with Senator Hirono out for surgery, does that mean instead of 50-50 plus Pence, they could get 50-49 or even 49-49 plus Pence with an abstention?

That is required by law for bills that pass under reconciliation.
(Also, for non-reconciliation bills that require 60 votes, the 60 is not affected by absent senators, as it is written as a percentage of duly sworn Senators. So only a vacant Senate seat would change the number.)
