Some questions about ACA open enrollment periods

I believe after the enrollment window closes on Monday, the next open enrollment is in November and only lasts 2 months, is this correct?

Does everyone need to go and re-sign up, or are those who are happy with their plans able to simply do nothing and let it renew automatically?

And logistically speaking, if someone misses the current period, they are out of luck insurance-wise until November unless they have a major life event such as the loss of a job or moving to a new state, but how difficult would it be for an insurance company to provide insurance for that time period, just not with all the protections of the ACA (for example, someone doing a routine enrollment outside the period would not be covered for preexisting conditions but would be covered for accidents, and would not get subsidies)?
