I wanted to address this to GD b/c this is where he posts.
Dirk is in surgery for a ghastly leg break (compound, pilon fracture: both tibia and fibula, with some joint involvement). He fell off a ladder this weekend -- it happened in a split second.
They've scheduled 2.5 hours for the procedure. He's about 30 minutes into it right now and I'm worried b/c I perceived a lack a coordination with his pre-op team...so maybe throw some of those positive thoughts my way too. I'm kicking myself that I didn't "interrogate" things a little further. I feel like if something happens it will be on me. Nurses seemed tired and confused about his anesthesia. Everyone looked harried.
His doc is amazing and I have the utmost trust in him -- this is his second surgery for plates and screws. I'm sure once the doc is there everything will be fine.
He hasn't posted much since the accident, but I want you to know that DU has kept him positive and engaged, and gave him something to focus on other than pain and worry. When he was getting extra anxious we changed the topic to the shutdown and how evil and stupid the Republicans are -- this had an amazingly good impact.
He values the community here and looks forward to recovering in a recliner with his iPad and good conversation in between trying to work from home.
I believe in the power of positive thinking...so, perhaps I'm really just trying to adjust my own attitude -- I left quite aggravated to come home and feed the dogs. His parents are in the waiting room. I just had to get it off my chest. This is going to be longest 2 hours ever.
update -- going back to the waiting room. he should be out in about 30-45 minutes! will take iPad for updates!
UPDATE 2 -- doc just came and said he's done and did great! two plates, one for tibia one for fibula. he has a nerve block that's going to keep him in the hospital another 24 hours. phew! He made it! no weird reaction to anesthsia...as the worst case scenario, that's therefore automatcally where i go
omg...thank you SO MUCH for the encouragement! i think i just got worn down!
UPDATE 3 -- post-op delerium...patient complains the Koch Brothers are keeping him from getting ice to make his throat feel better --

PS, I fell off a ladder during the summer of last year. Several broken ribs, they still hurt.

(20,958 posts)and they attached this CRAZY metal fixture that literally attached to the bones and tightened up on the outside -- kinda like a halo.
they had to let the swelling go down, which has taken 3 days...and they also gave him heroic amounts of antibiotics including vancomycin to make sure that anything he might have contacted with the open wound is beaten down into submission.
This surgery is for plates. if all goes well it will be his last. more surgeries will address anything that goes wrong...if anything goes wrong, of if he heals in a way that causes pain or disability.

(20,958 posts)all of his injury was down by the ankle. it was lighter weight, and kinda wrapped around to the back to the fibula.
and you're right! this is tricky surgery with a lot of possible complications including infection. his boot was hanging on just from flesh (ghastly!). i can easily imagine it being an amputation years ago.

10 minutes later i get a voicemail, in the calmest manner imaginable, "EMS is on the way. I'm lying in the driveway with a compound fracture. I'm not kidding."
Not a hint of panic in his voice -- just the facts.

then...every med student in the hospital came by to see the rare and spectacular injury. couldn't help myself...i took a few pics with the iPhone b/c i know...one day...he'll be able to look at that and say, "shit, i made it thru that!"

(20,958 posts)both bones mostly sheared away from his joint, which is good, but there's some damage. He's been wearing the external fixture for a few days, and the swelling finally went down enough to finish the surgery.
flying is going to be extra fun now!

The surgeon was super excited, because of the complexity of the break, it was the worst he'd seen. 17" scar. He was really jazzed about a doing complex surgery. I imagine yours will be as well.... if they cut through the muscle which they almost have to it will take about a 14 months to fully heal.
He'll be flyin' all right...

(90 posts)I broke my leg last November and the worst part was after the surgery. I was basically helpless. I couldn't get around much. I really learned to appreciate who my true friends were. Some family and friends just disappeared. When they saw me on crutches, all they would say was "You still not walking?!". What I'm saying is that recovering from a broken leg is not just physical, but also psychological.

(20,958 posts)while he's in the hospital we have the most help we'll have for the whole ordeal. As soon as he comes home...with no IV pain meds, and no extra hands to help...that's when the real work begins.
the psychological part is no joke. most people don't have a serious injury until they break something, so there's no sense of disability. then you realize how limited it makes your world. no stick shift car. no walks with dogs. no going into the office for a while (which has it's own set of psychological issues).
Today was the first day since it all happened that he said felt "optimistic." the sun was out, the pain was down, and he was looking forward to the final surgery. up until then he was beating himself up -- as if he had done something dumb. i can tell you it was a freak accident. i would have never left if i thought he was in any danger.

(20,958 posts)is like night and day. i've had hospital envy thru the whole thing. great docs. great nurse to patient ratio. they even started PT the very next day he was in -- no laying in bed. they made him get into a reclining chair and have him changing the leg position every hour or so. just remarkable care.
then i go to pre-op with him and all of a sudden i feel like i'm at one of my Democratic events where everyone is casting about before showtime, figuring out what's needed of them.
seriously...i think i drank too much coffee. they're on top of it.

I had 7 surgeries total and boy oh boy Doctors these days are amazing... took a long time and Dirk will go through a lot but will be OK!!
I can walk pretty normal, albeit with a big bad ass scar

(20,958 posts)i really can't praise it enough. especially after my own experiences with very bad care in a hospital in Nashville.
they have a commitment to pain management like no other i've seen. it's quiet and calm. clean and low nurse to patient ratio. there's amazing new resources...every two rooms have an IT station where the caregivers can review orders and enter new data. i'm really amazed for it being a suburban annex of Florida Hospital.

(20,958 posts)and... such a great team of nurses on his floor! the must be treating everyone well here bc the staff is AMAZING. i'm settled down now from my initial worry about the confusion in pre-op... dont want that to color the opinion of the hospital overall. its truly a great facilty.

(17,659 posts)If it's any comfort, he picked the right bones to break. NOT the calcaneous! He is luckier than you may imagine!
Seeing him well and you resting.
Just a thought, probably your docs don't send guys home without a reminder but if they don't give you a urinal, get him something handy to pee in from his bed. We learned the very hard way...my ohana passed out after going to pee, and he really didn't need the complications that followed.
Just for the first day at least!
The other suggestion I'd make, having just gone through a little nightmare in our ohana, is to make a chart for the pain meds that won't be confusing even when the reader is sleep deprived and on said meds, and keep a flashlight handy.

Reduce stress...breathe deep in 3 counts, hold 3, out 6 counts, repeat 10 times, you'll feel better!

(2,004 posts)Take care of yourselves and keep ahead of the pain once he comes home. My DH is recovering from a finger joint replacement and we found good pain management was critical. Get some help with the house work so you can get some rest. You will need it.

I don't like bad things happening to anyone, but especially not people who seem so nice. You don't get such a massive outpouring of support unless you really are. As for his delight in hearing the political news, tell him I said an enemy of my enemy is a friend of mine already.

But I totally did wake up with nasville_brook feeding my scorched throat ice chips, convinced we were post Koch-pocalypse rebels hiding out in some kind steam tunnels beneath Atlanta. Actually several minutes before I got oriented there. Many drugs affecting my perspective. And The post-op space in this hospital totally feels like a rebel base. Just like Revolution on tv, but more plausible.
I am lucky to be an insured American, getting good care from good doctors in a good hospital near home.
Luckier still to have NB watching my back.
Thanks for all the good vibes -- gonna get some sleep and do whatever they tell me to to start healing this up tomorrow.
Anyone have an opinion on a good knee sling / walker?

3.5 hours in surgery -- his surgeon was exhausted when he came out. here's hoping everything is put back together again!!

(15,480 posts)I had nearly 3 hours of surgery (which went well). My doc told me to take 6 weeks off of work (desk job) for recovery, but I'd told my bosses I was sure I'd be back in 2 or 3. I figured I'd be bored out of my mind at home and my job had little physical demand to it. Wrong. Something about the length of time under general anesthesia - left me weak as a kitten. I totally needed the entire 6 weeks. So take it easy on yourself - except when your physical therapist pushes you. PT is really critical and makes a ton of difference in recovering mobility/strength. Water walking is great for the legs, if you have access to a YMCA or other indoor pool.

You take it easy ok. And as the meds really wear off, the pain will hit you. Just flow with it. And after that, be a good kid and do your therapy religiously, even if at times it will hurt. Yup, that is from experience.

(2,899 posts)I was three months non-weight bearing on my right ankle after total ankle replacement. I loved my knee scooter and used it for over three months. Great around the house and shopping. I'm not sure it would be suitable for your surgery though. Here's the link to a knee scooter. It looks like the one I had. My insurance covered the rental;

Last year my brother broke his back and had to have it bolted and plated together. Very scary stuff but it all worked out.

(20,958 posts)he's really wanting to get out of the hospital -- but, I bet that as this block wears off and it's evident that the IV pain meds are needed, he'll cool his jets.
they have employed all sorts of amazing high tech stuff -- machinery to keep the leg cool with ice pumping thru the bandages and other stuff to keep blood from pooling in the other leg.

(8,303 posts)He's gotta stay put until they get his recovery to a certain point. You have to be able to move around enough not to develop blood clots.
In the initial stage, they often do all that you describe and then passive movement. I sympathize greatly, because I can't even drive past one without the urge to escape, but he needs to be there for a bit. Until he can move around on his own a bit without risk of undue swelling, he has to be there.

(2,979 posts)I had reconstructive surgery on my shoulderblade and collarbone a few years ago, it took 6 months to recover, your recovery will probably be at least that long. I have some friendly advice for you.
Don't get too happy with the pain meds,before I realized it, I was popping hydrocodones like Pez. I felt myself getting addicted and told my doctor, he reduced the dosage and strength of the pills gradually.
I still needed them for a while, until over the counter pain killers could do the job.

(20,958 posts)The pain meds enable us to "get back to it" quicker...which we need to do to get back to work...but it's all too easy to use them as a shortcut to that feeling of "wellbeing." when they're making you too "happy" it's time to dial them back and get thyself to PT.

this sounds much like my break of 2 years ago. Smashed my tibia and fibula- had to have an IM rod, 2 plates. My initial surgery was 4.5 hours. I had 2 subsequent surgeries and ended up with a year before the bones healed entirely.
but worse, I ended up with Complex Regional Pain Syndrome which I believe should have been caught much earlier. I complained about the symptoms for 3 months before anyone attributed it to anything but the breaks.
CRPS sucks and they say the earlier you catch it, the better. Not sure why that is, but that's what I've read.

(20,958 posts)This is something I've experienced since my osteomyelitis. You expect that once you're out of the woods, all patched and recuperated that you're done. But the body sometimes has a different idea.
These chronic pain syndromes are serious business and generally you don't get enough information about it when you're discharged. My experience was no pain management info from my ortho. No pain information from my GP. A nurse whispered to me that I needed to see a pain doc, as if it was a dirty secret.

(114,904 posts)My ortho guy is great but it still took him too long to send me to a specialist. If dirk feels any burning pain, pins and needles that won't abate, numbness, he should let his ortho guy/gal know and ask about CRPS which is different from most chronic pain syndromes and is not uncommonly the result of a traumatic injury. CRPS often isn't just about pain- changes in skin, disfigurement, etc.

Scary story.
I spent a lot of time on a ladder doing repairs and upgrades on our cabin getting ready for Winter. We will be on the roof cleaning out the stove pipe this afternoon.
Since we are currently uninsured, an accident like this could take everything we have.
It can and does happen in the blink of an eye.
We will be covered under the ACA (if we can ever get registered online).
Prayers/positive thoughts for Dirk right now,
and extra care working on the roof today.
