Let's Say a Woman Is Elected President or VP in 2016

How will she be addressed? I like the sound of 'Madam President' myself. 'Madam,' however, has some unsavory connections in our language, so the President might not like being called that. 'Mrs. President' wouldn't work if the woman is unmarried, and 'Ms. President' just doesn't sound right to my ears. Does anyone have other suggestions that might work? And just in case you're wondering, yes, this is the kind of question that I think about all day long.

'Madam' sounds so much better to me than the other options. I guess it's because it sounds more formal and respectful.

I'm interested in what the protocol nerds will do about Madam President's spousal unit:

President and Mr Smith or President and President Clinton or (your combo here)?

I have all sorts of names for Thatcher, but I might don't know what they called her before the queen gave her a title.

(ok, the rude language was implied but still)
