when he wanted a vasectomy several years ago (he is 54 now and lives in Orange Co, CA) his urologist refused to do it without the written consent of his spouse. He was unmarried at the time. So the compromise was that his live-in GF of a few months would sign.

Yes, an unmarried adult male needs the permission of someone not his wife to get a vasectomy in Orange County, CA.

I was about as astonished and outraged as I've ever been when he told me that.
