Simile uses "like" or "as":
Paul Ryan's words were like a vile stream of excrement spewing forth from the diseased bowel of a sick animal.
Metaphor just equates one thing with another symbolically:
Paul Ryan's actions were a slap in the face to charities and the people they help.
