DNR DNI

I do not have a record at the local hospital. Will my records be automatically transferred from old hospital? How can I obtain a DNR DNI document locally that would be legal? I doubt that the hospitals want us visiting just to sign a paper.

Let them know you would like to have the appropriate forms completed. Maybe they could mail them to you? You may need to sign them in front of a witness. Make sure to ask them where to send them after completion.
