NYT: Epstein taken off suicide watch after only 6 days. Who ordered that?

Jeffrey Epstein Dead in Suicide at Manhattan Jail, Officials Say
Mr. Epstein, the financier indicted on sex trafficking charges, was not under suicide watch at the time of his death.

Mr. Epstein hanged himself, the officials said. He was found at roughly 6:30 a.m. Saturday at the Metropolitan Correctional Center in Manhattan and was taken to a hospital, where he was pronounced dead, the federal Bureau of Prisons said in a statement.

Last month, a week after being denied bail on federal sex trafficking charges, Mr. Epstein was found unconscious in his cell at the jail with marks on his neck. Prison officials had been investigating that incident as a possible suicide attempt.

Mr. Epstein had been on suicide watch after he was found injured on July 23 and received a daily psychiatric evaluation, according to a person familiar with his detention. He was removed from suicide watch on July 29 and returned to the special housing unit, a segregated area of the prison with extra security, this person said.

The authorities did not immediately explain why he was taken off suicide watch. The F.B.I. said it was investigating, and Attorney General William P. Barr said in a statement that a special inquiry would be opened into what had happened.

ON EDIT:

Thanks to Claritie Pixie, who posted a link to the JD manual on this topic. I hope someone is filing for a FOIA for this required report :

Once an inmate has been placed on watch, the watch may not be terminated, under any circumstance, without the Program Coordinator or designee performing a face-to-face evaluation. Only the Program Coordinator will have the authority to remove an inmate from suicide watch. Generally, the post-watch report should be completed in PDS prior to terminating the watch, or as soon as possible following watch termination, to ensure appropriate continuity of care. Copies of the report will be forwarded to the central file, medical record, psychology file, and the Warden. There should be a clear description of the resolution of the crisis and guidelines for follow-up care.

At a minimum, the post-watch report will include:
? risk factors assessed,
? changes in risk factors since the onset of watch,
? reasons for removal from watch, and
? follow-up recommendations.

The suicide of the disgraced financier, who was found dead in his cell on Saturday, raised questions about what steps officials took after an apparent attempt in July.

But one federal prison official with knowledge of the incident said Mr. Epstein had been taken off suicide watch a few days ago, and was being held alone in a cell in a special housing unit...

Inmates can only be removed from the watch when the program coordinator, who is generally the chief psychologist at the facility, deems they are no longer at imminent risk for suicide, according a 2007 Bureau of Prison document outlining suicide prevention policies. The inmates cannot be removed from the watch without a face-to-face evaluation.

If he was placed on suicide watch only six days before, he must have -- or should have -- been evaluated. Sounds like the doctors and prison guards are being set up to take the fall.

So is it your opinion that jail cell murders do not occur? Are people never coerced into suicide when threatened?
Is all that just "silly speculation?" I know speculation may be taken too far - entering the realm of the absurd - but dismissing out of hand any speculation that there may have been bad actors in this seems disingenuous to me at least.

It seems to me that people in authority get away with terrible shit all the time.

I'm sorry that we can't give the lovely people of the corrections dept the benefit of the doubt. That seems to
hurt you. The shine is coming off many "professionals."

How could a psychiatrist have determined that a person who had attempted suicide was safe to go off the suicide watch, and be returned to the unit where he had previously attempted suicide, after only 6 days?

And with such a high profile prisoner, I doubt they left the decision up to a single individual. Someone made the decision and someone else approved it.

Who is that person?

DOJ manual for handling suicidal inmates clearly dictates the procedures, who is responsible for them, and that all actions must be documented.
