. . . . for anyone who is already in the system.?

That pause. It is part of every delivery of that line, no matter who is saying it. If one is a media editor, one can easily get a nice, clean cut of a repubican seeming to say "

The pause is most often taking a breath . . . . or the speaker seeming to be regathering one's thoughts . . . . or some other artful reason to stop the utterance for just long enough that one can assume a period to be at the end.

There is no period at the end of that part of the phrase. The sentence they are saying is "The repubicans have a plan to save Medicare without making a single change for anyone who is already in the system." That is a very different thought than what they can be made to appear to say.
