If you mean the ones with the pink and white ribbons, they are family orders. They are awarded by the monarch to close female family members at their personal discretion. QEII wears the family orders of her father (pink ribbon) and grand father (white ribbon).

They are not automatic and are awarded usually for service to the monarch. Princess Michael of Kent and Sarah, Duchess of York were never awarded the order despite being in the Royal family for many years.

The order is a picture of the monarch, usually surrounded by diamonds, hung from a watered silk ribbon - color changes with each monarch.
