Only the floor effected, and one to four floors above are typically asked to evacuate, and only if the fire is not contained quickly. Floors below are rarely asked to evac, and only when there is a likely concern that the fire is spreading in stairwells, vents, or elevator shafts. We drill on this four times a year in our building, but it is all commercial. Not sure if the rules are substantially different for a residential or mixed-use building.
