There is a psychological disorder known as BDD -Body Dysmorphic Disorder. Those with BDD are normal looking, even quite attractive in many cases, but perceive themselves as ugly and hideous.

I think there must be a corollary opposite disorder where people who look not so appealing (at least in terms clothing choices and freaky hair), look in the mirror and do not see what the rest of us see.
