Trump is mostly bald. He tries to conceal that with an "artistic" comb-over. It doesn't work. We've all seen his baldness. More importantly, Trump knows he's bald. There is no concealment possible for one's own knowledge.

There was a picture taken of him in a car (limo?) with glasses on. Other than that he's hidden it with notes written with a "20/20 pen" and other tricks.

He's as fake as they get.

2. He may know he's bald, on some level, but I suspect it isn't a conscious one

I worked with a pathological narcissist. She was middle aged and had, let's say, a short, thick and sturdy physique. None the less, she dressed like she was 24 years old, 5'8 and 100 lbs, and heading to the clubs. Most of her attire was in, one way or another, in violation of the company dress code but HR let it go because they didn't want to deal with her - she really was that frightening.

One day she showed up in a skin tight pastel colored dress and there wasn't just visible panty line - there was pretty much visible panty to the point you could easily discern the pattern. This was pointed out to her. She looked at her reflection in the mirror and vehemently denied what was so obvious to everyone. It wasn't what she wanted to see, so she didn't.

The bright side of that was, from that point on she could have walked out of the toilets with her skirt tucked into her knickers, and nobody would have said a thing to her.
