Since when did anti-Republican = anti-American?
I'm sure you all remember the "you're either with the Republicans or with the terrorists" mentality the right tried to perpetuate in 2003. How do they get away with claiming that they are the only Real Americans(TM) and that anyone who disagrees even a little bit is a communist and hates America? Hell, I remember some people here saying that they were called anti-American and even got death threats for having Kerry stickers in 2004.

6. Either way, that's when the big shift started happening.
The grand canyon sized divide in our nation. I know the groundwork was laid before that but when liberals started questioning the Bush admin after 9/11 something happened to turn the (R)s crazy.

3. I'm thinking the think tanks
The RW think tanks have been building up their ideology as the new normal for decades. Gradually they've moved the country more and more to the right, and they're not done yet. Any deviation from their far right point of view is cast as a manifestation of pure evil. But I've recently read David Brock's The Republican Noise Machine, and that may be shaping my understanding. It's probably more complex than this.
