Sweaty lazy-eyed Governor of Wisconsinstan Scott Walker, wants the names of outstanding public servants. He wants to (cough) recognize them (cough), probably with a pink slip or maybe a nice kick to the crotch.

Anyway, here's the form. After you identify yourself (expect a knock on the door in the middle of the night or a helicopter, probably) then you get to identify your nominee, who will probably vanish, too.
