1. Too much of a credentialing impulse
The undergrad urge for credentialing is ingrained in the college application process and they mistakenly think that it will matter to employers and grad schools too. In fact, you'd be better of focusing more on one area and really getting to know your profs who will be writing you letters and even networking on your behalf if you seem to hold promise.

One of his victims, Ryan Clark, had a triple major.

5. Seriously? I had a double in undergrad...
...and it wasn't too bad. Wouldn't have wanted a triple, but the option should certainly be available to those that want it. Most failed hyperachievers burn out only themselves, not others. And those who thrive on the challenge, well, more power to them. They sometimes gain a different set of priorities later in life (speaking from experience).

6. I attempted a double major.
Computer Science and Mathematics. I couldn't do it, and ended up with a Major/Minor. It's an incredible amount of work to pull off a double major, and I can't even imagine how hard that kid worked for a triple. What a god damned shame that he never got to enjoy the fruits of that work.
