It's an Axis-II disorder in the DSM-IV - effectively untreatable. It'd be my guess it's roots are in being an only child with with an absent father (alleged war 'hero,' BMOC, baseball 'star') and over-controlling mother from very privileged families. Upon the birth of his sister, he probably wished her dead. He got his wish. He discovered that all events confirmed his existence - an existence that was never verified by self-examination or internal reflection. This emotional imbalance was never addressed, only exploited and paved over with affluence in a dysfunctional family.
