Although the woman in the foreground is my sister, she is now a stranger to all who knew her and still love her.

3. I go for the first as well
I like both a lot, but the first one more fits the theme, as the detachment is more front and center.

4. At first I thought the second one, but then I thought the first one, but...
This is a really hard one. I like the direct look into the camera of the second, but I like the sharper background of the first which adds to the three-person portrait.

And the title, which ever photo you use, has to be: She is now a stranger to all who knew her.

5. Piping in once more: I agree with the title of you and NV Whino - and want to
say that the second one seemingly makes her look focused and "with it", which belies what you know. That's why I chose number 1.
