PC games with keyboard controls.
Hate them. I went out and bought Vampire The Masquerade: Bloodlines because I was in the mood for a dark, moody RPG. Opened it up and discovered that movement and most actions are keyboard controlled. It's like learning to type all over again. There is nothing worse to me than having to figure out what key to press to trigger an action when I'm in the middle of a battle.
I played it for a while and uninstalled it.

3. There was some mouse control
but most of the actions were keyboard controlled... too many, in my opinion.

Personally I can't control shit with gamepads. I'm great with keyboard and mouse. It comes naturally to me, even with sports games.

Gamepad...not so much.

I know there are plenty of people that are opposite of me, but I can't stand a game that forces you to use a gamepad.

And I don't mind some keyboard controls but when movement and many of your actions are keyboard-dependent it really bugs me.

5. I've been using the following for ten years

left hand: positioned with the thumb on the spacebar, the index finger on the 'D' key and the middle finger on the 'A' key, ring finger hovering over 'Shift' (my dash/speedburst button, nominally) and the pinkie finger on the 'CTRL' key (for reverse). The 'A' and 'D' are used for strafing left and right, and I bind other keys for the left hand as needed.

on edit: this allows my left hand to rest in a completely natural position while maintaining complete control over movement. I never actually have to adjust my fingers for anything; they're already in position.

For the right hane, RMB moves me forward, LMB attacks, and the scroll wheel cycles through weapons. The button in the wheel is used for alternate fire modes. I always have my mouse set to 'freelook'.

I've found this system of control works best for me for FPS games, but to each his own, I guess....

6. I'm a leftie, so I end up with a different setup anyway
I have LH-mouse, RH-NumKeyPad. Mouselook is always on, LMB is primary attack, RMB is jump (feels natural), mousewheel cycles weapons. I stay away from binding the mousewheel button to anything - it ends up being more of a hassle than a benefit. Keypad does strafe, Forward/backward, crouch, etc.

I have been using this for about seven years now and am quite comfortable with it.
