Why would my wife's taken-over (?) Hotmail account only send spam to four people?

It appears as though my wife's Hotmail account has been compromised, as I've been getting 1 or 2 spam emails from her address each week. However, these emails are only going to:

My main email address
My old work email address
Two of her friends

Why would it only send spam to 4 people from her address book instead of everybody?
