Obama does not make that clear here--unless we assume it is one of the choices in the 'Health Insurance Exchange.' I believe the HIE plan is in one of the House bills.

So we need to do a few things to provide affordable health insurance to every single American. The first thing we need to do is to protect what's working in our health care system. So just in case you didn't catch it the first time, let me repeat: If you like your health care system and your doctor, the only thing reform will mean to you is your health care will cost less. If anyone says otherwise, they are either trying to mislead you or don't have their facts straight.

Now, if you don't like your health care coverage or you don't have any insurance at all, you'll have a chance, under what we've proposed, to take part in what we're calling a Health Insurance Exchange. This exchange will allow you to one-stop shop for a health care plan, compare benefits and prices, and choose a plan that's best for you and your family -- the same way, by the way, that federal employees can do, from a postal worker to a member of Congress. (Applause.) You will have your choice of a number of plans that offer a few different packages, but every plan would offer an affordable, basic package.

Again, this is for people who aren't happy with their current plan. If you like what you're getting, keep it. Nobody is forcing you to shift. But if you're not, this gives you some new options. And I believe one of these options needs to be a public option that will give people a broader range of choices -- (applause) -- and inject competition into the health care market so that force -- so that we can force waste out of the system and keep the insurance companies honest. (Applause.)

I'm still on the fence on this particular bill because of salaries based on capitation are permitted and pharmaceutical care is limited to a formulary list with no express means to request/obtain a medical exception.

I like the idea of calling for a single payer plan - but permitting capitation based salaries provides a strong disincentive to deny visits to specialists and costly treatments/diagnostic tests. I don't understand why it is in an otherwise good (if vague) bill.

As someone who is currently incurring huge medical bills, I am thankful that I can live without the fear of financial ruin that many others must face. I can't imagine how I would feel if we did not have good insurance. There is enough stress in worrying about loved one's health concerns without the worries as to how the costs will be paid.

I am NOT signing this petition because it does not require:

That insurance companies accept all applicants for all plans. If insurance companies are allowed to pick and choose the public option will be a dumping ground for the most expensive patients; its premiums will necessarily be higher, and it will fail.

That both insurance companies and the public health option be prohibited from basing their premiums on the health of the enrolled individual

That there be subsidies based on income or a correspondence between premium levels and income so that the public health option is within reach of those who need it most.

Although single payer is my strong preference, I am will support a public health option that includes certain guarantees. Dean's terms can easily be satisfied by a public health option that is completely unacceptable and will fail. If it does, it will set back work towards real change at least a decade.

A public health option, alone, is not enough. At a minimum, it MUST require at least minimum affordability (subsidies or premiums based on income), it must not be competitively disadvantaged with respect to private insurers OR be permitted to adopt the bad habits of private insurers (both must be required to accept all applicants, and cannot be permitted to base the cost to the enrolled individuals on their health status).

WHEN Dean's petition expressly requires those minimums, I will sign it. I will not ask for something less than I consider a bare minimum to constitute real change.
