Just out:Cobra II, the Inside Story of the Invasion and Occupation of Iraq
The authors of this new book were interviews on NPR's "Talk of the Nation" today, the audio is linked below. They had a few very interesting callers too, one was a former Iraqi Translator for both this and the first Gulf War, and yes, they let one of those increasingly rare Bush die-hards thru too. They also wrote "The Generals' War : The Inside Story of the Conflict in the Gulf" about the 1991 Gulf War.

Talk of the Nation, March 14, 2006 · In their new book, Cobra Two, Michael Gordon and Bernard Trainor describe the Iraq War as a story of hubris and heroism, of high technology wizardry and cultural ignorance.

They conclude that the bitter insurgency that American and British forces confront today was not pre-ordained, and they recount the military and political opportunities that were lost along the way.

1. Keith Olbermann had a segment on this book tonight
They discussed the general that almost got canned for predicting the insurgency right after the fall of Baghdad.
