1. Yes, I've used them within the last two years.
I think I got them at walmart.

4. I still find them at the fabric store, and if you don't find any...
Stitch-witchery and a piece of any cloth works great. Also activated by an iron. I love that stuff.

6. Would you say Stitch-witchery works better? I agree with ThomasQED that patches peel

after several washings, probably due to the dryer heat, though I dry everything on the lowest setting.
Next time I use an iron-on patch I'll try hanging the pants to dry.

7. Let's just say that I have a hippie maxi skirt from the 60's with
a stitch-witchery patch (on the inside), and it has been washed hundreds of times. It's still holding. I think it may depend on the fabrics used and how well you melt the stitch-witchery. Make sure you get it all the way to the edges, and use a paper towel between iron and jeans.

5. Yes, but...
they don't stick on very well.

I tried that (can't sew) and they all started peeling up from the edges within a few washes. At that point I sewed the edges and that worked... but doesn't look so great.

9. Sew them with decorative stitches in contrasting colors
And they will look cooooool - just like my jeans from the 60s did!

8. You need to air dry the jeans after you patch... avoid hot water as well
anything hot will loosen the patch over time!

10. And here I thought that the holes in clothes...
had become fashionable. ;^)

Glad to hear that they are still available.
