1. *spoilers*
He was crazy. Right from the beginning. he was an inmate they tried to go to great lengths to rehabilitate, including allowing him to think he had some command over the people around him, to freely roam across the island in search for what he thought was the truth.

At the end, I think he decided to go with the doctors to get the lobotomy because, deep down, he knew he was sick and that he would never fully be well. "is it better to live as a monster or die as a good man?" On some level, he knew he was a monster, and that he wanted to die as a good man, even if it meant losing a chunk of his brain.

At least, that's what I took away from the movie.

4. He was nuts. And he was willingly going for a lobotomy at the end of the movie.

5. Thanks, I've been saved a Netflix use.
I've been hesitant about renting it. From the trailer, I suspected he was crazy.

7. The trailer was almost TOO revealing
As soon as they said in the trailer that there was an extra patient, I KNEW it was going to end up being him. I just didn't know how it would be revealed in the movie.

6. He was "cured" at the end
But chose to pretend he was crazy so that he would be lobotomised and not remember what had happened.
