Found my e-mail address on a hacker website, and was wondering how concerned I should be.

why was it there?

2. seems to be a list of webmail addresses and some "code" for each one

I don't know if they'd just use it for spamming, or attempted ID theft, or what... someone was kind enough to PM me and offer to take a look at the page, so we'll see what his diagnosis is.
