Well, I just did that.

I'm on a listserv, hit reply to send a message to one person. The message may or may not have included the term "shirtless pirate".

Imagine my surprise when I received my message a couple seconds later from the listserv and realized it went out to everyone.

1. I've done that. Posted a personal email to a newsgroup.
Luckily I caught it right away and I think only one person saw it before I got it removed.

MAJOR sense of panic when I realized what I'd done.

2. This one has already been sent to everyone, unfortunately
Thankfully, I'm none too embarrassed by the email, even including the shirtless pirate part, and I'm willing to take the lumps when I start getting laughed at.

3. Never did it myself, but was on a listserv where it happened
This was about 11-12 years ago, I believe. There were maybe 50 active members of the list that was related to a sports team, so everybody knew everybody fairly well, including spouses and kids, etc...and there was a yearly get together for one of the games.

But, a guy that was one of the more outgoing members of the list sent an email to the list that was obviously intended for somebody else, as it was a PG-13 level romantic type email and their plans for their romantic getaway together.

The worst part was that it was addressed to a woman that was not his wife. One woman replied and said, "isn't your wife's name ____?" Nobody else said anything to the list.

Needless to say, the guy was offline for a while.
