It was one of the "Truth" anti smoking ads (featuring popcicles laden with glass shards).

Is that an "AD Council" ad or privately funded?

I didn't see it on TV, but from what I've read online (here, mostly), it was obviously deliberate. It's not entirely clear whether CBS knew about this beforehand, though.
