To me, it always sounded like "Physician at Medicine."

1. There are lots of attorneys who do not practice.
An "Attorney At Law" is a practicing attorney, as opposed to one engaged in another form of employment.

Hmmm. Good point. Even if they're not practicing law, I guess they'd still be about the business of the law.

Maybe an actual attorney can enlighten us....sure hope so, because this is the kind of thing that keeps me up at night. :o

3. Do you have to renew licenses as attorneys?
I might guess that the "at law" means you are up to date.

Of course, that would probably apply to physicians as well...

5. Every year we send in a few hundred dollars to the State Bar
and have to complete 12 hours of classes ("continuing legal education hours").

That renews our license.

another person's Power of Attorney.

6. There are attorneys that are "of counsel"
there to advise members of the office, but that do not practice law.

Practicing attorneys, general private, one man offices, are referred to as "Attorney at Law".

"at Law" doesn't bother me, it is the "Esquire" title that lawyers have assumed.
