Questions about the DEBATE

On a few news shows last night, I heard the commentators asking the question if the presidential debates will actually take place. Are there conditions where the debates would NOT take place? I know who the mods are, but are the questions pre-selected? Who determines what questions are asked? Do the candidates get to ask each other questions? If the mods determine the questions, do the candidates know in advance which specific questions will be asked? Do the mods make sure the candidates get specific with their answers, or do they accept generalities (such as terrorists attacked us because they hate our freedom)?

I have seen some presidential debates, but I've slept since then, and have never given them alot of thought before.
