Who gave order to wait until after the 2004 election to attack Fallujah?

This documentary describes the US use of phosphorus bombs on the citizens of Fallujah:

Toward the end of the documentary, the question was asked if the US waited until after the 2004 election to attack Fallujah. One of the ex-Marines interviewed indicated that this was true. They waited until after the election results to invade. It sounded as if invasion/attack plans were "on hold" and the officers were anxious to invade and didn't want another Bush v Gore showdown. Upon learning of the "official" results, the US instantly attacked.

My question: who would have given the order to hold off until after the election results were "official?" Rumsfeld? Cheney? Bush? Chairman of the Joint Chiefs of Staff? Who would have given the initial order?
